ITP staff redeployed to the Community Response Team
Recently a group of our team has been redeployed to the Community Response team. The CRT team works within the Discharge to Assess Pathway. This pathway merges health and social services together to allow for a smooth discharge for patients from hospitals across Norfolk.
For our team this was something very different then their normal roles within ITP. The team would liaise with the hospital when somebody was due to be discharged and then coordinate their discharge by organising social care if required and ensuring therapy was provided when needed.
Like with anything recently all of this must be done remotely, so our teams have had to learn a whole new way of working across multiple platforms.
In more recent weeks some our of team have started to return to ITP and have reflected on their time working away from ITP.
‘My redeployment into D2A has provided me with an insight into the invaluable role provided by this service with essential, effective and timely multiagency working between the NHS and Adult Social Care. It’s a highly responsive service which enables earlier discharge of patients with appropriate support of packages of care and responsive, timely therapy assessments.
Prior to my position in the role I had no idea what POC even stood for! We were welcomed warmly into the team and gained great support throughout. Training provided was excellent, we always knew who to turn to for help and advice as our manager was fabulous; it was never an issue if we made a few mistakes on the way! It was a pleasure to help the service within a great, supportive team during a difficult time.’
