Update on the Education Triage Scheme

You will herewith find details of an offer from Norfolk Community Health and Care NHS Trust (NCH&C) Neuro-Developmental Service (NDS). We would like to  work collaboratively with schools/colleges to identify children and young people that may benefit from an onward referral to our diagnostic pathway.

The NDS service is available for consultation direct with SENCOs.   If you wish to make a referral for a child where there are concerns regarding a CYP’s profile of skills and behaviour which may indicate a possible Neuro-Developmental condition, please ask your SENCO to ring  01553 668712.  When you call, our administration team will book you a consultation slot; slots will be 30 minutes long, for each child to be discussed. Consultation slots are available daily from Monday 5th September 2022 (term time). Attached are some template forms to guide you with gathering the types of information we would need to support the discussion. You will need to gather information from your setting, the family and the family GP (if the GP is involved). Please do not submit this information to us prior to the appointment, it is to aid the discussion and can be used to support an onward referral if this is what is agreed.

The Consultation Line is staffed by experienced Clinical Psychologists, Therapists and Nurses who will be able to advise you on how to make a full referral  to NDS, if that is appropriate.  They will be able to suggest alternative avenues of support or assessment if a referral is not felt to be appropriate.   A key benefit for the family and SENCO is that for many cases raised via the Consultation Line,  it will not be necessary for the referral to NDS to be made via the GP.  Referrals can be made, after accessing the Consultation Line, direct to NDS.

The NDS service is provided by NCH&C NHS Trust and therefore is not available to schools outside the NCH&C area.

Neurodevelopmental Service

This multi-professional service assesses children where there are concerns regarding possible neurodevelopmental disorders such as Autism Spectrum Disorders (ASD) and Attention Deficit Hyperactivity Disorder (ADHD).

 

Who is the Neurodevelopmental Service (NDS) for?

Pre-six assessment service

All under 6s will need to be seen by our Community Paediatricians first before referrals are sent to the Pre-Six Neurodevelopmental Pathway.  This is to ensure a thorough medical assessment is completed and available to NDS staff when they meet with you.

The child’s assessment could include:

  • A visit at home
  • A visit in nursery/school
  • A clinic-based assessment
  • Parents/carers and nursery/school may be asked to complete questionnaires

Members of the pre-six assessment team include:

  • Paediatricians
  • Clinical Psychologists
  • Speech and Language Therapists (include representation from East Coast Community Health Care, ECCH, Speech and Language Therapy Service)
  • Occupational Therapists
  • Assistant Psychologists
  • Family Support Workers

Every child will be discussed at a multi-professional panel meeting throughout their assessment and when a final conclusion is made, you will be offered a feedback appointment with a Paediatrician or a member of the NDS team.

Post-six assessment service

Once received, referrals will be triaged by NDS clinicians and a letter will be sent to the referrer and parents to inform of the outcome.

The child/young person’s assessment could include:

  • Initial telephone consultation
  • A visit at home
  • A visit in school/college
  • Clinic-based assessments
  • Video assessments
  • The young person, parent/carer and school/college may be asked to complete questionnaires

Members of the post-six assessment team include:

  • Clinical Psychologists
  • Specialist Nurses
  • Speech and Language Therapists
  • Occupational Therapists
  • Assistant Psychologists
  • Family Support Workers

Every child/young person will be discussed at a multi-professional panel meeting throughout their assessment and when a final conclusion is made, you will be offered a feedback appointment with a member of the NDS team.

 

Referrals

Pre-Six Neurodevelopmental Service

All children referred into this service need to have been seen by a Community Paediatrician. Find out more.

All children need to be referred in using the single point of referral (SPOR) referral form. Referrals can be made by GPs or Schools. Find out more.
